Indications/Uses
Acute episodes & maintenance treatment of schizophrenia. Acute manic episodes w/ bipolar I disorder.
Dosage/Direction for Use
Schizophrenia Initially 10 or 15 mg once daily. Maintenance: 15 mg once daily. Bipolar mania Initially 15 mg once daily, may be increased to 30 mg once daily based on clinical response, adjusted at intervals not <24 hr.
Administration
May be taken with or without food.
Special Precautions
Discontinue use if NMS, tardive dyskinesia or impulse-control problems occur. Hyperglycemia & DM. Orthostatic hypotension, known CV disease eg, history of MI or ischemic heart disease, heart failure or conduction abnormalities; history of seizure or conditions lowering seizure threshold eg, Alzheimer's dementia; body temp regulation; patients at risk for aspiration pneumonia; suicide. Pathological gambling, hypersexuality & impulse-control problems eg, personal or family history of obsessive-compulsive, impulse-control & bipolar disorders, impulsive personality, alcoholism, drug abuse or other addictive behaviours. Avoid overheat exposure & dehydration; alcohol. Concomitant use w/ prescription or over-the-counter drugs. May affect ability to drive & use machines. Not to be used during pregnancy & lactation. Paed <18 yr. Elderly ≥65 yr.
Adverse Reactions
Headache, dizziness, akathisia, somnolence/sedation, tremor; blurred vision; tachycardia; orthostatic hypotension; nausea, vomiting, constipation, dyspepsia; asthenia/fatigue; NMS, tardive dyskinesia, seizure, cerebrovascular adverse events, increased mortality in elderly demented patients, hyperglycaemia & DM.
Drug Interactions
Enhanced effect of certain antihypertensive agents. Increased clearance & lowered blood levels by CYP3A4 inducers eg, carbamazepine. Inhibited elimination & increased blood levels by CYP3A4 inhibitors eg, ketoconazole or CYP2D6 inhibitors eg, quinidine, fluoxetine, paroxetine. Concomitant use w/ alcohol.
